The primary function of the RN Case Manager is to plan, organize and coordinate client care responsibly for all assigned cases, with an emphasis on community health education/experience. The RN Case Manager will utilize resources of the community to develop, implement, and evaluate individual client care plans and update as needed according to established professional standards of care. The RN Case Manager will serve as a liaison between the agency and the client or client's family in order to meet their needs within their home or communities.


1st shift


Must have current physical examination showing freedom form certain types of communicable disease

Must have current RN license with the State of Kansas

Must have a valid driver's license

Prefer a minimum of three years' experience as a licensed, registered nurse, including at least one year of experience in home health

Must have a current CPR card issued by a recognized provider such as American Heart Association or American Red Cross

Must have previous experience working in an office environment gathering information, preparing documents and managing records

Must have intermediate working knowledge of Microsoft products such as Outlook, Word and Excel


Medium Work - Exerting 20 to 50 lbs. of force occasionally, and/or 10 to 25 lbs. of force frequently, and/or greater than negligible up to 10 lbs. of force constantly.
