RN Case Manager - Hospice

Status: FT and PT positions

Job Description


The primary function of the Hospice RN Case Manager is to plan, organize and direct hospice care and will be experienced in nursing, with emphasis on community health education/experience. The Hospice RN Case Manager builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.


Primarily 1st shift


Must have current physical examination showing freedom from certain types of communicable diseases.

Must have Registered Nurse license issued by the State of Kansas.

Must be a graduate of an accredited school of nursing with one (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care or hospice.

Must have a current Kansas driver's license, auto insurance in accordance with state requirements, and vehicle in good working order.

Must have current Cardio Pulmonary Resuscitation Card (CPR) issued by recognized provider such as American Heart Association or American Red Cross that includes hand-on practice and in-person skills assessment.

Must have previous work experience in an office environment gather information, preparing documents, and managing records in an accurate and efficient manner.

Must have intermediate working knowledge of Microsoft Products such as Outlook, Windows, Excel and Word.


Medium Work - Exerting 20 to 50 lbs. of force occasionally, and/or 10 to 25 lbs. of force frequently, and/or greater than negligible up to 10 lbs. of force constantly.
